Toya Crutchfield
Clinical Director, PT

Physical Therapy was a second career for me, but I always knew I wanted to be a PT.  I was fortunate enough to have some great physical therapists take me under their wing.   I  believe we change and improve lives. I am totally committed to this profession and my patients.  I attended the University of TN and I have been a PT since 1999. I love it more every day.  I was born and raised in Memphis, TN. I have lived in Bradenton since 2003.  I love living and working in this community.  The people here are great and I am so grateful to live this close to some of the most beautiful beaches in the U.S.

I enjoy gardening and cooking for my family.  My partner and I have two boys and a daughter. I very much enjoy being a mom and going to sporting events with  my kids.  I have always been an athlete and I played short stop in softball, basketball, racquetball, I competed in barrel horse racing.  I’m very happy to be part of Transitions Rehabilitation and excited for our future.
