Transitions Rehabilitation offers multiple plans to employees and their families depending on health needs and financial considerations.  Our plans are in compliance with the Affordable Care Act for affordability and value. You have a large network of doctors, specialists and other providers to choose from without necessary approvals for most visits.  Affordable copays for visits and prescriptions.


An indemnity plan is available with discounts available if you stay within the network of dentists and providers in your area.  Family coverage options are available. No cost basic cleanings available twice per year.


A network of vision providers and clinics for you and your family provides quality eye care at a very affordable cost.

Voluntary Life

Term life coverage is available for employees, spouses and children. The benefits may be taken with you on an individual basis if you decide to leave employment.

Short-Term Disability

In the event you are unable to work due to a medical disability, this benefit  provides partial income replacement while you are unable to work.
